In order for them to not leak gas, come loose, or otherwise induce malfunctions, one has to ensure that it's accurately trued to the carrier and that the correct screws are used (and properly staked in place). Most of the time many hope that the manufacturer has taken care of this–but it isn't exactly uncommon for an end-user or armorer to replace screws or be more aggressive about the staking. VooDoo Innovations solved that issue with their new Integral Direct Impingement Carrier.
